JCAHO Right of Notification of Safety or Quality Concern

CMH Regional Health System is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). JCAHO standards relate to hospital quality and safety. Clinton Memorial is committed to meeting JCAHO standards and will have an unannounced survey by the JCAHO at least every three years to assess our compliance to the standards.

If you have any questions, comments or concerns about care received at our hospital, please discuss your concerns with the caregiver, department manager, or Leslie Nahigyan, R.N., Performance Improvement Director, at 937-382.9335.

You are always free to contact JCAHO directly to raise your concerns if they are unresolved by hospital staff.

If you have questions about how to file your complaint, you may contact the Joint Commission toll free at 800.994.6610 from 8:30 to 5 p.m., Central Time, weekdays.

JCAHO will treat your name as confidential information and not to disclose it to any other party, though it may be necessary to share the complaint with the subject organization in the course of a complaint investigation. JCAHO policy forbids accredited organizations from taking retaliatory actions against employees for having reported quality of care concerns to the Joint Commission.
